CPAPsupplies.com Loyalty Rewards Program — Full Terms and Conditions

By enrolling in the CPAPsupplies.com Loyalty Rewards Program, you agree to the Loyalty Rewards Program Terms and Conditions outlined below; you agree to CPAPsupplies.com’s Website Terms and Conditions; you agree to the Promotions & Coupons Terms and Conditions. If you do not agree with all elements of the three listed sets of Terms and Conditions, your account may be subject to forfeiture and/or cancellation. For purposes of this agreement, “CPAPsupplies.com,” “us,” or “we” shall mean CPAPsupplies.com and any of its predecessors, successors, assigns, parents, subsidiaries, affiliates, and each their respective officers, directors, shareholders, employees, and agents. These Terms and Conditions do not alter the terms or conditions of any other agreement you may have with CPAPsupplies.com for other products or services.

CPAPsupplies.com reserves the right to change the Terms and Conditions of the Loyalty Rewards Program at any time without notice, including the right to discontinue or change the benefits of the Loyalty Rewards Program. If we make any material changes to these terms and conditions or terminate the Loyalty Rewards Program, we will post the new terms at CPAPsupplies.com Terms & Conditions page. All Loyalty Rewards Points and subsequent Rewards must be earned and used according to the terms and conditions of the Loyalty Rewards Program as of the date the Points are earned. Loyalty Rewards Points or subsequent redeemable discounts cannot be applied to any previous or pending purchases.

The CPAPsupplies.com Loyalty Rewards Program is a free customer loyalty program that allows participants to earn points (“Points”) for every dollar spent on Eligible Product Purchases (as defined below). The Loyalty Rewards Program is free to enroll. To enroll and become a Member, an individual must create a customer account at CPAPsupplies.com. Points accrue when a Member’s unique Loyalty Rewards Program Member Number is provided at the time of purchase for both in-store and online Eligible Product Purchases. Under the Loyalty Rewards Program, “Eligible Product Purchases” means purchases of CPAPsupplies.com products such as CPAP replacement supplies, new supplies, masks, OSA treatment accessories, and CPAP cleaning supplies, subject to any exclusions applicable at the time of sale or as stated herein. Members also have the ability to periodically earn bonus Points. CPAPsupplies.com has the sole right to make the final decision on whether a purchase qualifies for Points (or an action qualifies for bonus Points) under the Loyalty Rewards Program. No adjustments to Member’s Account will be made after 90 days from the actual purchase date.

As a Loyalty Rewards Program Member, the Member earns one (1) Point for every $1.00 spent on Eligible Product Purchases. Once a Member reaches 100 points, they are able to redeem those points for a $5 discount. Redeemable points exist in tiers of 100 points, at which the Member will receive $5 off for each incriment of 100 points. Please note, the maximum points that a Member can redeem per transaction is 500 points, regardless of the total number of points within a Member's account. 

At incriments of 100 points, Members are eligible to redeem their points for tiers of $5 off eligible purchases. Please note, there is no limit on how many Loyalty Rewards Points you can earn. However, there is a limit on the amount you can redeem per transaction. The limit of points you can redeem in one transaction is 500 points, which equates to $25.00.

Rewards are automatically added to Member’s account once earned and 100 Points are automatically deducted for every $5.00 Reward issued to Member’s account.

Point calculations are based upon dollars spent at checkout on Eligible Product Purchases less any dollars spent on sales tax, shipping charges, delivery charges, restocking fees, rebates applied at the time of purchase, the dollar amount of available mail-in rebates at the time of purchase of any product or service, the dollar amount of the purchase of Gift Cards, and the dollar amount of any Gift Card offer with purchase or other excluded charges.

Members do not earn Points for the amount of any Reward redeemed or discounts applied to an Eligible Product Purchase. When Members return products, Points previously posted to Member’s Account for that purchase will be deducted from Member’s Account. Other exclusions may apply. Points and Rewards have no cash value, are nontransferable, except as specifically permitted in connection with certain promotions, and are not valid toward previous purchases, pending purchases, gift cards, or third party payments, such as Affirm. Points may not be combined among Members or conveyed by any means to anyone, including through a Member’s estate and may not pass to Members’ successors and assigns. Points do not constitute property of the Member. CPAPsupplies.com may revoke some or all Points and Rewards if it is determined, in CPAPsupplies.com’s sole discretion, that a Member received Points and Rewards due to an error, through fraud, abuse, or deception, or in any manner not authorized or intended by CPAPsupplies.com.

CPAPsupplies.com is not responsible for communications, including Rewards Points, lost due to communication issues including change of address or other contact information or technical issues or malfunctions. Points that remain on Member’s account two (2) years from the date earned will be forfeited. If the Loyalty Rewards Program is terminated or the Member’s account is closed, blocked, or deleted, any remaining earned Points will be forfeited. Rewards are for one-time use only. In the event that a Member returns merchandise that was partially or wholly paid for with a Reward(s), the Reward(s) used in connection with such a purchase shall not be reissued and no Points representing such Reward(s) shall be reissued to such Member’s Account. In these cases, the Member forfeits the Rewards(s). Rewards are not redeemable for cash and no change or currency will be given for Reward redemption. Members may be notified of additional ineligible products and services. Other restrictions may apply. Lost, stolen, or expired Rewards will not be replaced.

The Loyalty Rewards Program is available to individuals who are at least 18 years of age. The Loyalty Rewards Program is intended for personal, non-commercial use only and is limited to one account per individual. CPAPsupplies.com may refuse to create a Loyalty Rewards Program Member account for any reason. CPAPsupplies.com reserves the right to exclude any individual from or discontinue any individual’s participation in the Loyalty Rewards Program. CPAPsupplies.com reserves the right to cancel or suspend participation in the Loyalty Rewards Program in the event of fraud, abuse of Points or Rewards privileges, violation of these Terms or CPAPsupplies.com’s Terms of Use or any applicable law, or at CPAPsupplies.com’s sole discretion. Any Points and Rewards in the account will become void at the time of cancellation or suspension.
